Teen Confirmation (Grades 9-12)

How to Register:

Please download the registration forms below and return the completed forms, required certificates, and full payment to the Faith Formation Office on one of the following dates:

Sundays; 9:00 AM to 2:00 PM (by appointment). Monday – Thursday; from 12:00 PM to 6:00 PM (walk-in):

July 19 to 23, 2026
July 26 to July 30, 2026
August 4 – August 9, (by appointment only)

Please Have Ready:

Completed registration forms (Teen and Family info and emergency)
Copies of certificates: Birth, Baptism, & First Communion
Completed Sponsor form
Full payment due

Please contact the Office of Faith Formation prior to the registration deadline if you are not able to submit payment in full and arrangements are not made before the deadline; late fees will be applied.

Dates to Remember:

Registration Deadline: August 24, 2026
Parent Orientation: August 30, 2026 @ 2:00 pm (Confirmation Calendar will be provided)
First Sessions @ 3:30 pm in Quinn Hall:
- Confirmation 1, Sept. 20th
- Confirmation 2, Sept. 13th

Requirements:

Participation during sessions (classes)
Participation during Mass
Attendances, 2 absences.
Attend Retreats (one per year)
20+ Service Hours (Faith, Church, & Community)
Parent and/or Sponsor Volunteer Hours (i.e., serving together at our Church Festival)
Pre-Confirmation Interview with Year 2 Candidates
Safe Environment Training: In compliance with the Diocesan requirement for Child Protection education, a formation meeting will be dedicated to presenting topics relating to health and safety issues each year.
